Job description for Community Nurse (Daycare) at NTUC Health
Job Brief
The Community Nurse is responsible to provide a high standard of care through care management, assessment, planning, implementation and evaluation of nursing care to meet the needs of the clients, including educating family members. Furthermore, they are responsible to provide cost-effective care in accordance to established policies and procedures, standards of nursing care and practices.
Primary Responsibilities and Duties (80%)
- Complies with approved standards of nursing practices
- Responsible for assessing the needs and delivery of quality geriatric / dementia nursing care to clients
- Perform pre-assessment of new clients refer by AIC
- Ensure the clients’ enrolment is comply with the Company’s SOP
- Administer the documentation to be submitted to relevant government body
- Assess and coordinate with health care team to assess, plan, implement and evaluate client’s conditions
- Monitor, assess and review clients’ progression and implement care plan
- Provide health promotion, counselling to clients and families
- Consult and coordinate with health care team to assess, plan, implement and evaluate client’s conditions
- Perform nursing care such as medication administration and wound dressing and other technical skills as required
- Oversee and manage the performances of the nursing team / care staff to ensure desired level of work competency
- Coordinating, implementing and monitoring clinical and service quality performance indicators for continuous improvement plan
- Perform CPR, AED and first aid to clients and plays an active role in emergency situations.
- Assist and support delivery of health promotion activities in the Centre.
Secondary Responsibilities and duties (20%)
- Participates in clinical service projects
- Participates in infection control campaign activities.
- Assumes additional responsibilities as assigned by Senior Nurse Manager.
- Participates in training and development of care staff.
- Demonstrate accountability for one’s development and status as a professional Registered Nurse.
- Other duties as assigned by the SNM / Center In Charge
- 5 day work week (shiftwork)
Qualifications and Education Requirements
- Registered Nurse with Singapore Nursing Board
- Degree or Diploma in Nursing
- Advanced / Specialist Diploma in Geriatric / Community Nursing an added advantage
- Minimum 3 years of relevant hospital work experiences is preferred
