Job Description
Roles & Responsibilities:
CASEWORK AND COUNSELLING
Manage 25-30 C&C active cases (mainly low-moderate level of complexity) per quarter.
Conduct assessment, planning and intervention for cases by taking into account the integration of a range of information, including practice guides and organisational frameworks.
Conduct case conferences with partners.
Participate in networking and case management meetings.
Document and evaluate case outcomes.
Develop and review appropriate intervention strategies.
Ensure proper documentation of cases.
Ensure reports on cases are submitted in a timely manner.
INFORMATION AND TRIAGE
Manage case referrals.
Part of the Intake & Triage Team to conduct intake assessments for referred clients based on the practice standards of the Code of Social Work Practice and Practice Guide.
Refer clients to appropriate agencies for further intervention.
COMMUNITY WORK
Network with other organizations and stakeholders.
Participate in implementing and evaluating of community outreach / initiatives.
GROUP WORK
Be part of the programme team for Group Work conducted at the FSC.
REQUIREMENTS
Degree or Graduate Diploma in Social Work or related field such as Psychology, and other relevant fields in Social Science
Previous experiences via internships or placements in related areas of work will be considered
Understanding of social work values and ethics
Understanding of work with vulnerable populations i.e. low-income families
