The incumbent is part of Special Needs Group who reports to the Head of Enabling Services Hub (ESH) and is responsible for the role of Social Worker I that encompasses implementing holistic services and programmes that will benefit and make an impact to the needs of clients and caregivers  under ESH and Special Needs Group. The incumbent is responsible for delivery of quality service, personal efficiency and teamwork at the workplace. 

Case Work / Information and Referral

• Conduct assessment and/ or intervention of clients and caregivers 
• Support the referral of services for Persons with Disabilities (PwDs) and/ or caregivers 
• Provide service/care coordination of PwDs and caregivers from information gathering, intake assessment and case coordination with other stakeholders/ agencies when required 
• Casework management for ESH clients who need support such as social-emotional support, long term care planning etc. 
• Handle enquiries pertaining to services and to assess potential clients for intake to ESH or other services under TOUCH Special Needs Group 

Community Outreach and Programme Support

• Assess and recommend programme offered by ESH to PwDs and caregivers. Conduct feedback and evaluation for PwDs and caregivers attending ESH services. 
• Outreach to community partners to advocate for Persons with disability and recruitment of potential ESH clients.  
• Outreach to PwDs and caregivers in the community through various means such as door-to-door or roadshows etc. 
• Build and maintain strong relationships with families and caregivers and organize and conduct caregivers’ support activities under Special Needs Caregivers’ Support Group. 
• Support and/or observe client’s participation and/or behaviors in programme under ESH 
• Work with staff from another service under Special Needs Group or with other services outside Special Needs Group on special project on ad-hoc basis. 

Administrative Duties

• Produce & maintains reports for documentation and submission to funders. 
• Produce & maintain proper social report and case notes recording under case management. 
• Draft articles on clients for newsletters or publicity when needed. 
• Be part of the team in implementing strategies to create awareness and publicity of the Special Needs Group. 
• Degree holder or Graduate Diploma in Social Work 
• Registered Social Worker under Singapore Association of Social Work 
• Preferably with 2-5 years of social work experience in working with persons with special needs/ disabilities. 
• MS Excel, MS Doc. Intermediate skill in MS presentation 
• Planning/ Organising – Is able to organize own work effectively, creates own work schedules, priorities, prepares in advance and sets realistic timescales. Has the ability to visualize a sequence of actions needed to achieve a specific goal and to estimate the resources required
• Teamwork – Works co-operatively and flexibly with other members of the team with a full understanding of the role to be played as a team member to achieve a common goal. 
• Working with Family – Work in consultancy with family members that will uphold TCS’s vision of building strong families, caring generation and enabled communities.
