Deskripsi pekerjaan Senior Staff Nurse (Home Care) Assisi Hospice
Reporting to the Nurse Manager (Home Care), you are responsible for delivering patient-centric and compassionate care according to Home Care patient’s needs and condition, in compliance with organisational standards, statutory requirements and stakeholders’ expectations.
Responsibilities:
Deliver safe, competent and individualised nursing care in accordance with established standards, including documentation, treatment, patient monitoring and infection control
Manage complex patient care and demonstrate sound clinical judgement, critical thinking and professional competence
Assess, monitor and evaluate patients’ conditions, and implement appropriate care plans while supporting patients and families coping with advanced illness
Coordinate patient care in collaboration with the attending physician and multidisciplinary team, and act as a liaison between patients, families and healthcare professionals
Provide direct patient care, including medication administration, wound care and other nursing interventions as required
Assess patients’ home care needs, including caregiver training and the provision of equipment and assistive devices
Ensure continuity of care through timely follow-up and coordination of treatment plans
Maintain accurate and timely documentation of patient care, including clinical records and service data (e.g. referrals, enquiries, attendances)
Participate in multidisciplinary team meetings and collaborate with internal and external stakeholders to ensure smooth care delivery
Ensure patient safety at all times and adhere to infection control and clinical governance standards
Comply with organisational and regulatory requirements, including proper handling, storage, and control of medications and medical supplies
Maintain strict confidentiality and uphold professional and ethical standards in patient care
Ensure proper use of equipment and responsible utilisation of consumables and resources
Handle enquiries in a professional and appropriate manner
Adhere to organisational policies and procedures, including the Singapore Nursing Board’s Standards of Practice and Code of Ethics
Keep up to date with industry developments and continuously upgrade clinical knowledge and skills
Maintain a clean, safe, and well-organised work environment
Requirements:
Diploma in Nursing from a recognised institution and registered with the Singapore Nursing Board
Working knowledge of IT systems and electronic medical records
Strong interpersonal and communication skills, with the ability to engage patients, families, and stakeholders
Meticulous and detail-oriented, with good clinical accuracy
A collaborative and effective team player with a positive work attitude
Passion for patient care and service excellence
Able to work effectively in a fast-paced and dynamic environment
