The Agency for Integrated Care (AIC) aims to create a vibrant care community for people to live well and age gracefully. AIC coordinates and supports efforts in integrating care to achieve the best care outcomes for our clients. We reach out to caregivers and seniors with information on staying active and ageing well, and connect people to services they need. We support stakeholders in their efforts to raise the quality of care, and also work with health and social care partners to provide services for the ageing population. You will part of the Supportive Care team to contribute to the formulation, elaboration, and implementation of care services for community care sector in Singapore

This role involves policy analysis, stakeholder engagement, and operational planning to ensure services are client-centric, sustainable, and aligned with national healthcare strategies.

  • Lead comprehensive service reviews to conduct landscape scans, identify overlaps, and pinpoint gaps within the community care landscape.
  • Collaborate with internal and external stakeholders to pilot, operationalise, and support enhancements or adjustments to community care services, providing ongoing assistance to designated providers post-implementation.
  • Execute programme evaluations to assess service suitability for mainstreaming of services where applicable.
  • Develop, implement, and refine standard operating procedures and workflows to ensure alignment with policy objectives.
  • Conduct performance management activities, reviewing and tracking indicators relevant to community care services.
  • Continuously review and optimise care-related processes within community care services to uphold established care standards.
  • Evaluate work processes to drive ongoing improvements in service quality and grant governance.

Job Requirements

  • Degree in any discipline
  • Experience in community care, service development, or healthcare policy.
  • Experience in stakeholder engagement and project management.
  • Strategic thinker and self-directed
  • Excellent written and verbal communication skills
  • Willingness and skill in working across teams, divisions, disciplines, and organizations to achieve shared objectives.
  • Ability to attend to different initiatives within the same time period and monitor timelines to ensure respective initiatives' progress are on track
