Responsibilities:
1.Care Navigation & Integrated Service Delivery
Support the coordination of referrals, assessments and follow ups for seniors and caregivers.
Assist in developing care plans and maintaining continuity of care across services.
Work with internal teams and partners to support preventive health activities, outreach efforts, and data-driven interventions.
2.Team Development & Systems Management
Support onboarding and coordination of Care Navigators and micro jobbers, including scheduling of training and activities.
Assist with CRM updates, case tracking, and documentation to ensure information flow and service continuity.
3.Stakeholder Engagement & Community Activation
Support partnership activities and help coordinate workflows with community partners.
Assist AAC teams in outreach, programme delivery, and initiatives that enhance community engagement and senior participation.
4.Performance Monitoring & Continuous Improvement
Support KPI tracking, reporting, and documentation of outcomes.
Surface ground issues and contribute to enhancing workflows and service delivery.
Carry out other duties as assigned to strengthen care integration and community engagement.
Requirements:
Diploma or Degree in a healthcare, social sciences or community-related field
Preferably 1 - 2 years’ experience in health, social, or community care settings.
Experience in care coordination, community engagement or programme is preferred.
Good communication and organizational abilities
Able to manage ground operations and support care coordination processes
Proactive, adaptable, and able to work collaboratively with internal teams and community partners
